To be completed by all First Responder students when beginning a FR course. This application should be sent to KBEMS by your instructor with a $10 application fee. A $15 fee will be required at the end of class to obtain your KY FR certification.

Please be aware that a criminal background check from state of residency for past 5 years will be required to be submitted with all FR initial applications.
