How to Become an Educational Institution

The Educational Institution Packet is for agencies wishing to obtain approval to give any kind of EMS classes in the Commonwealth of Kentucky.  A money order or certified check payable to Kentucky Board of EMS must be included for the amount of $250.00.  For any questions, please contact Jon Muncy by email at jon.muncy@kctcs.edu or telephone at 859-256-3565.

TEI Checklist (PDF): All requirements to be met to apply for an Educational Institution. 

TEI Application (PDF: Application to be completed to apply to become an educational institution.  Please check all areas that you think you may ever want to offer in your agency.  Only submit the syllabus for each area that you are wanting to start immediately.  You can send in the other syllabi as you are ready to start teaching in those areas. 

TEI Agreement (PDF): The agreement that you are making with the Kentucky Board of Emergency Medical Services that you understand the regulations and agree to follow the regulations that are approved.  This document must be notarized before sending to the KBEMS office.
