Below you will find the documents set forth by KRS 311A.055 when making a complaint against any provider or individual licensed or certified by the board. Additionally, you will find a form that provides a mechanism to make a complaint against any KBEMS staff person. Both forms must be filled out in its entirety and submitted to the office for investigation.

Complaint Form (PDF) – This form allows an individual to file complaints against EMS Provider/Agency, Ambulance Service, Fire Department, EMT, Paramedic, First Responder and Educational Institutions.
Staff Complaint Form (PDF) - This form allows an individual to file complaints against Kentucky Board of Emergency medical Service Board Members or Kentucky Board of Emergency Medical Service Staff.

Public_Notice_of_Negative_Action.doc- Pursuant to 202 KAR 7:201 Section 7, 202 KAR 7:301 Section 7, 202 KAR 7:401 Section 9, 202 KAR 7:501 Section 16 and 202 KAR 7:510 Section 14 the KBEMS office shall cause to be published, in the KBEMS News or similar publication of the board, or otherwise disseminate the name of a first responder, EMT, paramedic, ambulance provider or medical first response agency that is fined, placed on probationary status, placed on a restricted status, suspended or had a certification or license revoked.
